When to Consider Extraction

Your dentist will typically recommend extraction when one or more of the following apply:

Ongoing pain or jaw stiffness that does not resolve between episodes — usually from an impacted tooth pressing against the second molar in front of it.

Recurring gum infection (pericoronitis): bacteria trapped under the gum flap of a partially erupted wisdom tooth cause swelling, pain, and sometimes pus discharge.

Decay on the wisdom tooth or the adjacent second molar: the position of a partially erupted tooth makes reliable daily cleaning difficult.

Impaction or misalignment:the tooth is angled toward a neighbouring tooth, disrupting your bite or undoing previous orthodontic treatment.

Cysts found on X-ray imaging: fluid-filled cysts form around impacted teeth and erode surrounding bone over time.

If none of the above apply, your dentist may recommend monitoring with a check-up every 6 to 12 months rather than immediate removal.

Here’s a step-by-step overview of a wisdom tooth extraction procedure at Casa Dental:

Your dentist will conduct a comprehensive oral examination, take dental X-rays, and when needed perform a CBCT 3D scan to map the exact position of the tooth and surrounding nerves. This allows us to plan the safest extraction path and discuss your sedation options. If we identify any underlying issues such as decay in a neighbouring tooth, those will be addressed before extraction.

You will receive specific instructions about fasting, medication, and what to bring. Follow these closely they directly affect how smoothly your procedure goes.

  • If nitrous oxide sedation is planned, avoid eating or drinking 2 to 4 hours before your appointment. Continue taking regular medications with a small amount of water unless your dentist advises otherwise. Avoid alcohol for at least 24 hours before the procedure.
  • Clear Liquids: Some offices may allow clear liquids (anything see-through) up to 4 hours before surgery, but this can vary.
  • Regular Medications: Continue taking your regular medications with a small amount of water unless advised otherwise. If there are any changes in your medication or health (like a cold or fever), please let your dentist know.
  • Avoid Alcohol: It’s best to avoid alcohol for at least 72 hours before your wisdom tooth extraction procedure, as it can interfere with your anaesthesia.
  • Refrain from smoking for at least 12 hours before your wisdom tooth extraction surgery.
  • A responsible adult must accompany you, stay during the procedure, and drive you home afterwards. This is crucial due to the effects of anaesthesia, which can impair your judgment and motor skills for up to 24 hours after surgery.
  • Set up a cosy recovery area at home, stock up on soft foods (like yoghurt, smoothies, and mashed potatoes), and have cold packs on hand to manage any swelling after your procedure.

The dentist will numb the area with local anaesthesia. If you have chosen nitrous oxide sedation, it will be administered through a small mask. For impacted teeth, a small incision is made in the gum to access the tooth. Deeply embedded teeth may be divided into smaller sections to minimise trauma to the surrounding bone. Once the tooth is out, the site is cleaned and closed with dissolvable stitches in most cases so no return visit is needed for stitch removal.

You will rest in a recovery area until you are alert. We provide a care kit (gauze, prescribed medications, and written instructions) along with a follow-up date. Most patients are home within 30–60 minutes of the procedure ending.

A follow-up visit is typically scheduled 7 days later to monitor healing, check the gum tissue, and remove any non-dissolvable stitches.

How long does wisdom tooth extraction take to heal?

Soft tissue heals within 1 to 2 weeks. Bone remodelling takes 3 to 6 months, but most people eat normally and return to their routine by day 10 to 14. Simple extractions: 1 to 3 days of soreness. Surgical removal of an impacted tooth: 5 to 7 days.

What can I eat after wisdom tooth extraction?

 First 24 to 48 hours: porridge, yoghurt, mashed potato, scrambled eggs, smooth soup, applesauce. From day 3, add soft pasta, tofu, steamed fish. Skip hard, crunchy, sticky, and spicy foods for at least 7 days. No straws, the suction pulls the blood clot out and causes dry socket.

Can I brush my teeth after extraction?

Yes. Brush your other teeth as normal from day 1. Leave the extraction site alone on day 1. From day 2, brush the area gently. Rinse with warm salt water 2 to 3 times a day from 24 hours after the procedure. No rinsing on day 1.

How do I know if the extraction site is infected?

In normal healing, pain and swelling reduce day by day through the first week. Call your dentist if you notice pain getting worse after day 3, fever above 38°C, pus or a bad taste at the site, swelling spreading toward the jaw or neck, or difficulty opening your mouth. Do not wait for a scheduled follow-up.

Can I claim Medisave for wisdom tooth extraction?

Yes, for surgical extractions. Medisave covers wisdom tooth surgery at Ministry of Health-accredited private clinics. Casa Dental is accredited. Simple (non-surgical) extractions are not claimable. CHAS, Pioneer Generation, and Merdeka Generation subsidies apply for eligible patients. Your dentist confirms eligibility and explains the claim process before the procedure.

Do I need antibiotics after extraction?

Not always. Your dentist prescribes antibiotics based on the extraction’s complexity, your medical history, and your infection risk. Many patients having straightforward removals do not need them. If you are prescribed antibiotics, finish the full course even after you feel better. Do not use leftover antibiotics from a previous prescription without asking your dentist.

Is it normal to have a fever after extraction?

A temperature below 38°C in the first 24 to 48 hours is part of normal healing. Fever above 38°C, or any fever appearing after day 2, signals a possible infection. Call your dentist that day. Same-day review is also needed for: pain that keeps worsening, pus or a bad taste at the site, and swollen lymph nodes under the jaw.

How long does bleeding last?

Light bleeding or oozing is normal for up to 24 hours. Bite down on gauze for 30 to 45 minutes after the procedure. If heavy bleeding continues past 24 hours, fold a fresh gauze pad, bite down for 30 minutes, and call your dentist. Spitting, rinsing hard, and straws all restart bleeding, avoid all three.

Does wisdom tooth extraction hurt?

Local anaesthesia numbs the area before any work begins. You feel pressure and movement, not pain. If sharp pain breaks through, tell your dentist, they will add more anaesthesia on the spot. Soreness after is normal for 2 to 4 days. Paracetamol or ibuprofen handles it for most people.

How long does pain last?

Soreness peaks at 24 to 48 hours and clears within 3 to 5 days for simple extractions. For surgical removal of impacted teeth, expect up to 7 days. Pain that gets worse after day 3, rather than better, points to dry socket or infection. Call your dentist that day.

How long does swelling last?

Swelling peaks at 48 to 72 hours and settles over 5 to 7 days. Cold compress for 15 to 20 minutes at a time through the first 24 hours. From day 3, switch to warm compress to bring the swelling down. Some patients bruise on the cheek alongside the swelling. Both clear within a week.

What is dry socket and how do I prevent it?

Dry socket happens when the blood clot at the extraction site comes out before the wound heals, leaving bone and nerve exposed. Pain starts 3 to 5 days after surgery, throbs, and often radiates to the ear or temple. To avoid it: no straws, no smoking, no spitting, no hard rinsing for at least 48 hours. Smoking is the highest single risk factor. It dislodges the clot and cuts blood flow to the healing site.

How many MC days will I get?

Simple extraction: 1 to 2 days. Surgical removal of an impacted tooth: 3 to 5 days. Most people in desk roles are back within 2 to 3 days. If your work is physical, take the full 5-day MC for surgical cases. Exertion raises blood pressure and dislodges the clot.
